device loses ability to transmit frames

Occasionally, my AR5005G disconnects from the network and is not able to reconnect. In this state, the driver and device state seems fine, and you can fire off all the usual ioctl's to request scanning, association, etc. However, association always times out, it's impossible to get reconnected.

A neighbour device in monitor mode shows that the device is not transmitting any frames at all, but I inserted some trace points in the driver and it does appear that all is well at the driver level, the driver believes it is transmitting just fine.

When the device is in this state, there is no known solution other than rebooting the system: bringing the interface down/up does not help, neither does reloading the module.

This is with madwifi- and has been reproduced on several different boards. I have a suspicion that switching networks is one way to reproduce this, however my test system just survived 36 hours of switching between 3 networks with no problems.

Sorry for the lack of specificity, I don't have any details recorded, but I just wanted to get this bug on record for now. I will add further details next time I see it happen.
